Care-Plan Sample Language for a Trans Resident
Drop-in care-plan language for identity, personal care, personal items, hormones, and documentation — using the resident’s own words.

Adapt the bracketed fields to the resident. Use the resident’s own words wherever possible. Keep legal-name data in a separate administrative field.
Identity
Resident’s chosen name: [name]. Pronouns: [pronouns]. Lived gender: [gender]. Legal name on file: [legal name] — used for pharmacy, billing, and legal documents only. All verbal and written communication, door, wristband, and whiteboard use the chosen name and pronouns.
Room placement
Resident is accommodated according to lived gender. If no appropriate shared space is available, a private/single room is used as a temporary bridge pending a suitable bed.
Personal care
Resident prefers [a woman / a man / no preference] for personal care. Words the resident uses for their body: [words]. Cover or handle [item] first. Causes dysphoria or discomfort: [details]. Approach: offer the most private option available, extra coverage, and follow the resident’s lead on timing.
Personal items
Resident wears [binder / breast form / wig / packer]. Worn: [when]. Cleaned: [method]. Stored: [location, named container in room]. Night plan: [details]. Handle with care on every shift and every service (housekeeping, laundry briefed).
Hormone therapy
Current hormone regimen: [medication, dose, route]. Prescriber: [name, contact]. Regimen continues unchanged until a prescriber directs otherwise. Responsible prescriber going forward: [name] — confirmed by [date]. Medication review scheduled: [date].
End-of-life
At death, use chosen name and pronouns in all communication and in dressing the body. Call: [chosen family / SDM]. Death certificate legal-name fields completed per provincial process. Team debrief to follow.
