Home care is one-to-one in a client’s home, where staff confidence and clear expectations matter most. Add these to your intake and service agreement.
Intake questions
Add voluntary, private questions: chosen name and pronouns; lived gender; who matters to the client and who is involved in care decisions; personal-care preferences (including caregiver gender); anything about their body or personal items staff should know.
Service-agreement clause — identity
"We will use the name and pronouns you provide in all communication and documentation. Your identity information is private and shared only with the staff caring for you. You may change this information at any time."
Service-agreement clause — caregiver matching
"Where possible, we will match caregiver gender to your preference for personal care. Every caregiver on our team provides respectful care to every client."
Service-agreement clause — chosen family
"We welcome the people you identify as family — including chosen family — in your care, and we will involve them in care decisions as you direct."
Staff briefing
Before the first visit, brief the assigned caregiver on the client’s name, pronouns, and care preferences. Confirm the client’s identity information is not shared beyond the care team.
